Information We Ask For
We may ask for basic contact information, state, approximate incident timing, lawyer status, type of help needed, and a short description of what happened.
Information Not To Send First
No medical records are needed in the first request. Do not send Social Security numbers, insurance IDs, payment information, full dates of birth, private account logins, or complete legal files.
How Requests Are Used
We use the first request to understand what kind of resource, funding support, legal direction, or next-step help may be relevant. The public form is meant to collect enough information to start a careful review without asking for sensitive files immediately.
Submission Security
The intake form is prepared for secure delivery and uses required consent, hidden-field spam checks, timing checks, server-side validation, rate limiting, and sensitive-identifier rejection. If stronger spam verification is enabled later, the form may also use an anti-bot challenge provider.
